[ARCHIVED] FREE flu shots for all City of Hampton employees

Register Online

You will need to complete the Immunization Consent form prior to receiving your flu shot. You can complete this form ahead of time which will shorten your wait time the day of the shots. If you wish to have a copy of this form for your records, you will need to complete 2 copies of the form and bring them both with you on the 10th. Maxim will need to retain one for their records.

Please note that they will NOT run out of shots, so you do not have to be here right at 9:00am. That tends to be the busiest time, so you should anticipate a wait time if that is when you decide to come.

If you wish to set up an appointment online you can do so by going to https://maximflu.bioiq.com and following the instructions to the right. The online registration closes 24 hours before the event begins. You are not required to set up an appointment, however Maxim will honor appointments first and accommodate walk-ins where space is available. This should help to relieve some of the wait times we have experienced in the past. Also, if you print out the completed consent form (see below), that will also cut down on your wait time.
