[ARCHIVED] Change of mailing address request form for real estate and personal property

This form must be completed and returned to the Office of the Assessor of Real Estate by an owner, corporate officer of the entity that owns the property, or an authorized agent: Authorized agents will need to provide a copy of a power-of-attorney document or a completed Agent Authorization form from the Office of the Assessor of Real Estate.
