City Manager's Message Sept. 17: Vaccination status form and weekly testing

Sept. 17, 2021 — I am writing with an update on our efforts to begin mandatory weekly Covid-19 testing next month. With so many moving parts, we are still finalizing the logistics which has pushed back our start date to Monday, Oct. 11. Several of you have contacted me directly with specific questions and it is our intent to include them in our FAQs. However, there is some information that I can share with you now, but I again ask for your patience as the program is finalized.  We have purchased self-administered Covid-19 test kits which will allow the testing to take place in multiple city buildings instead of employees scheduling appointments at pharmacies and/or medical offices.  In order for us to identify the buildings, we need to get an approximate count of the number of employees who need testing and their work location. 


The link below will take you to a form that will require you to declare your testing status.


This form can be completed using any computer or mobile device. If you are fully or partially vaccinated, you must upload proof of your vaccination. Your response and any proof of vaccination will be housed in a secured portal that is compliant with the Health Insurance Portability and Accountability Act (HIPAA).  Once we obtain this information, we can identify the number of tests needed and assign departments to specific testing locations.  Every employee is required to complete the form and there is even a box to select if you decline to answer. If you received your vaccination in Virginia you can click on the link below to access an electronic version from the Virginia Department of Health’s Vaccination Portal.


Employees have until 11:59 p.m. on Friday, Sept. 24 to complete the form.  If you or your co-worker needs assistance in completing the form, please notify your supervisor.
